Below is the file you will need access in order to fill out paperwork before beginning your community service hours. Click on the file icon to download. Please fill out all information asked and sign and date paper work then mail back with a check or money order made out to ECOSLO.
Keep your time sheet separate and take with you to the place you volunteer. Make sure to have all necessary initials and signatures along with case/ docket number. Finally, mail or fax your completed time sheet in for reporting.
